There is a section to complete your personal details on the front and the Terms of Employment can be found on the middle two pages. On the back of the booklet there are various declarations to sign (some may not be applicable) to say that you have understood and completed the contract terms and all supporting paperwork e.g. health and safety handbook, P46 form.
If you are claiming expenses from the first week, you must also submit an expenses form for any allowable business expenses that you may have incurred.
